When you first enquire about BBMP, you may be asked to complete an enquiry form by the website. When making initial appointments you are asked to complete a consent form requesting private details and sensitive information. Face to face or via telephone or videolink conferencing, more detailed information is collected verbally and during physical examination. You may also provide information when paying accounts, or making complaints.
All the information that is collected electronically, is stored in an Australian Based software system, Better Clinics that is a cloud-based practice management system that upholds the requirements for online safety and security to hold your information safe. Better Clinics complies with the Privacy and confidentiality act.
Any paper documents are stored in a lockable filing cabinet, accessible to relevant practitioners only.
What information we collect
The following information is deemed necessary to collect to provide the required health care that is sought by our clients.
Identification details such as Name, DOB
Contact details such as phone number, email address, postal address
Information about key contacts or NOK
Government identifiers such as Medicare no., DVA no.
Contact information for other Health Practitioners involved in your care
Sensitive information about your health status, symptoms, medical history, social , personal and employment history relevant to your case
Results of relevant investigations
Whilst you do not have to give us these details, providing such information helps us to provide the best quality of health care.
Collection of information from minors
Relevant information may be requested in order to provide best possibly care to children under the age of 18 when;
A parent or guardian is present
Or with their parent or guardian’s full consent
All information collected on minors is stored in the same manner as described in this policy.
Why we need to collect that information
In order to provide the best possible care we need to be able to communicate to you, manage your health and also liaise to other health practitioners involved in your care.
